Additional Purchase Form Use this form to make additional contributions to a PA 529 Investment Plan account. (Do this online.)
Automatic Investment/Electronic Bank Transfer Form Use this form to start, change, or stop automatic bank transfer contributions or to add or change bank information for electronic contributions to a PA 529 Investment Plan account. (Do this online.)

Annual Exchange/Future Contribution (Allocation) Form Use this form to move money within a PA 529 Investment Plan account to a different investment option or to change investment options for future contributions (allowed anytime). (Do this online.)
Account Information Change Form Use this form to change your name, address, phone number, account owner, successor account owner information, or to add an interested party to a PA 529 Investment Plan account. (Do this online.)
Beneficiary Change Form Use this form to change the beneficiary on an existing PA 529 Investment Plan account or to transfer a portion of the account assets to a new beneficiary's account. (Do this online.)
Withdrawal Request Form Use this form to request a withdrawal from a PA 529 Investment Plan account. (Do this online.)
Employee Payroll Deduction Form Use this form if your employer offers payroll deduction and you want to have money automatically deducted from your paycheck and deposited into an Investment Plan account, or if you want to change your existing deduction. (Do this online.) (If you're opening a new account, you can enroll online and select payroll deduction as your contribution method. Or include your payroll deduction instructions on the Enrollment Application.)
Form W-9 Request for Taxpayer Identification Number and Certification Use this form to certify the account owner's or beneficiary's taxpayer identification number.
Limited Power of Attorney/Agent Authorization Form Use this form to grant another person or organization limited authority to act on your PA 529 Investment Plan account(s). This means that the person or organization may receive certain account information or take certain actions that you specify on the form.
Power of Attorney Form Use this form to grant another person or organization complete authority to act on your PA 529 Investment Plan account(s). This means that the person or organization may receive any account information or take any action that you, as the owner of the account, could receive or take.
Organization Resolution Form Organizations designated as an agent on a Power of Attorney form or a Limited Power of Attorney/Agent Authorization form must use this form to identify the individual(s) who is authorized to receive information or take authorized action on PA 529 Investment Plan account(s) on behalf of that organization.
